The serpentine mitral valve and cerebral embolism
نویسنده
چکیده
Valvular strands, well-delineated filiform masses, attached to cardiac valve edges are associated with cerebral embolism and stroke. Strokes, caused by emboli from valvular strands, tend to occur among younger persons. In this case report a valvular strand, giving a peculiar serpentine appearance to the mitral valve is described. This mitral valvular strand was the only explanation for an episode of cerebral embolism, presenting with a transient right sided hemiparesis. It is proposed that a randomized study involving combined treatment with aspirin and clopidogrel is warranted in young patients with valvular strands, presenting with a first episode of cerebral embolism.

Myotonic dystrophy, mitral valve prolapse, and cerebral embolism.
CARDIAC abnormalities are the rule in hereditary myopathy, and myotonic dystrophy (MD) is no exception. Conduction disturbances are most common, manifested by first degree atrioventricular block, intraventricular conductive defect, and by evidence of disease in the entire conductive system on His bundle electrogram. The association between mitral valve prolapse (MVP) and MD has been reported as ...
متن کاملTranscranial Doppler application for risk assessment of stroke in patients with mechanical mitral valve
Introduction: Observing microembolic signals in brain vessels by transcranial Doppler technology is a very common finding in patients with mechanical heart valve. These signals are almost always clinically asymptomatic. The aim of this study was assessment of transcranial Doppler in detecting probability of stroke in patients with mechanical heart valve. Methods: This descriptive study was ca...
متن کاملInter-Operative Time between Neurosurgery and Cardiac Surgery for Native Valve Endocarditis
Introduction: Infective endocarditis is well known to produce neurologic complications from septic emboli, which mandates a multidisciplinary approach between neurosurgery and cardiac surgery. However, literature is not clear as to the optimal interval length between neurosurgery and cardiac surgery.Presentation of Cases: The first case involves a 60-year-old woman with significant past medical...
متن کاملMitral valve prolapse complicated by acute cerebral embolism, arrhythmias and painless myocardial infarction. A case presentation and overview.
A case of 'primary' mitral valve prolapse is documented. The patient was admitted with right-sided hemiplegia of sudden onset, probably caused by a cerebral embolus from the mitral valve. He also had a painless transmural inferior myocardial infarction (MI) of indeterminate age which was diagnosed electrocardiographically and on left ventricular cine angiography. Since selective coronary arteri...
متن کاملAssessment of mitral valve function at operation and the prevention of systemic embolism.
When assessing the function of the mitral valve at operation by palpation or inspection, the surest way to prevent systemic (cerebral) embolism is to have the ascending aorta occluded by a clamp. A technique is described which allows safe assessment of mitral valve function by aortic root and left ventricular perfusion, giving a beating-heart preparation with the aorta cross-clamped. This metho...
